DEADLINE EXTENDED TO JANUARY 31, 2011.   YOUR RESPONSE IS NEEDED URGENTLY.

From Mr. Robert Fukuda, Executive Director, USJF to Dojo Senseis:

We are aiding researchers Dr. Rainier Ganschow/University Hospital Hamburg, Dr. Yukio Oku/University Hospital Hamburg, Dr. Yuji Ozawa/Kumamoto University. and the German Judo Federation in the collection of data for their research on injuries sustained during judo activities.

Thus, please find attached a copy of questionnaire for the collection of data for the research. We would greatly appreciate it if you would please distribute this questionnaire to your students and instructors and encourage them to complete and submit the completed questionnaire.

Completed survey forms may be submitted to:

1. You. dojo sensei,  as a collection point and forwarding to our offices

2. USJF offices for collection (USJF National Office, PO Box 338, Ontario, OR 97914-0338

3. The researchers directly via the email address on the form

We would like to submit all of the questionnaires we receive to the researchers in January 2011. Thus, we would appreciate it if you would please request everyone to submit his or her forms by the end of the year.

We hope to provide them with the large sample size that will give their study greater validity. If you have any questions, concerns, or suggestions, please do not hesitate to call the USJF National Office at (541) 889-8753.
